Job summary

CenterWell Home Health is seeking a Patient Services Coordinator-LPN to schedule visits and coordinate care with field staff, patients, physicians, and the care team. The role supports daily office operations and staff management while ensuring continuity of care.

The position requires an active State LPN/LP license, at least 1 year of home health experience, and strong communication and organizational skills. A valid driver’s license is required. Comprehensive benefits are provided.

Qualifications

• Must be a Licensed Professional Nurse or Licensed Vocational Nurse licensed in the practicing state.
• Minimum 1 year home health experience.
• QI experience preferred.
• Coding certification preferred.
• Valid state driver’s license and automobile liability insurance.
• Currently licensed in the employing state if applicable.
• Excellent communication, strong organizational skills, and ability to multitask.

Responsibilities

• Schedule visits for all patients and coordinate with field staff, patients, and physicians.
• Edit schedules when staff are unavailable and reassign patients in a timely manner.
• Process infection control forms and route to HRD for uploading in the worker console.
• Back up during peak times and assist with calls from field staff and case conferences.
• Maintain hospitalization and coordination logs; ensure notes and logs are shared with relevant teams.
• Schedule and verify visits, add-ons, OASIS collection, and discharge visits.
• Reschedule missed or declined visits and manage reassignment tasks.
• Ensure supervisory visits are scheduled and review weekly schedules for productivity.
