Job summary

Humana’s CenterWell Home Health is hiring a Patient Services Coordinator to schedule visits, coordinate with field staff, patients, physicians, and other stakeholders to ensure care continuity. The role supports office operations, staff scheduling, and infection control processes.

Qualified candidates will have strong communication, organization, and multitasking abilities with at least 1 year of home health experience. Coding certification is a plus.

Qualifications

• Must possess excellent communication skills and the ability to interact with diverse groups.
• At least 1 year of home health experience.
• Prior packet review / QA experience preferred.
• Coding certification is preferred.

Responsibilities

• Schedule visits for patients and coordinate with field staff, patients, physicians, and other stakeholders.
• Manage and edit staff schedules, reassign when needed, and update agent unavailability.
• Perform back-up coverage during lunch hours and busy periods; assist with weekly case conferences.
• Initiate infection control forms and route reports to the appropriate departments.
• Run scheduling reports and prepare weekly staff schedules for supervisor review.
• Assist with transfers of patients between branches and support office operations.
