Job summary

Humana Inc. is seeking a Patient Services Coordinator-LPN to manage schedules for patients, coordinate with field staff and physicians, and support daily office operations across the local CenterWell Home Health team.

This role emphasizes care coordination, timely updates to the scheduler, infection control documentation, and the ability to handle high patient volumes while maintaining compassionate service.

Qualifications

• Must be a Licensed Practical Nurse (LPN) licensed in the state.
• At least 1 year of home health experience.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Excellent communication and organizational skills.

Responsibilities

• Schedule visits for patients and coordinate with field staff and physicians to maintain care continuity.
• Initiate infection control forms and upload to the worker console as needed.
• Serve as a back-up during lunch hours and busy times, handling calls from field staff.
• Maintain the patient hospitalization log and share coordinates with staff.
• Schedule add-ons and orders, including discharge visits and OASIS collections.
• Run scheduling reports and prepare weekly schedules for Branch Director approval.
• Verify visit notes in scheduling console and assist with transfers between branches.
• If clinical, review lab reports and route to Medical Records; may perform patient visits or on-call rotations.
