• Manage the intake process for new patients, including gathering demographic, insurance, and clinical information.
• Coordinate with referral sources such as hospitals, physicians, and discharge planners to facilitate smooth patient transitions
• Verify insurance eligibility and obtain authorizations for home health care services
• Communicate with patients, families, and caregivers to explain services and set expectations
• Maintain accurate and up-to-date patient records in the electronic health record (EHR) system
• Collaborate with clinical and scheduling teams to ensure timely start of care
• Follow up on pending referrals and track intake metrics to support agency growth
