Job summary

Apploi is seeking an Intake Coordinator for CHHA services to serve as the primary contact for patients, families, referral sources, hospitals, and physicians. You will coordinate intake and onboarding from referral through start of care, ensuring timely communication and accurate documentation.

You'll verify Medicare/Medicaid/HMO eligibility, coordinate with clinical teams, and maintain compliant records in EHR systems, contributing to smooth admissions and high-quality patient service.

Qualifications

• Minimum of 1 year of experience in home care intake or healthcare administration.
• Knowledge of CHHA regulations, Medicare, Medicaid, and HMO requirements.
• Excellent verbal and written communication and strong interpersonal skills.
• High school diploma or GED required; associate degree preferred.

Responsibilities

• Receive, review, and process referrals from hospitals, physicians, and insurance providers.
• Verify insurance eligibility and obtain prior authorizations.
• Coordinate start of care with clinical teams and schedule assessments.
• Maintain accurate patient records in EHR and ensure regulatory compliance.
• Collaborate with intake, scheduling, and operations to support onboarding.
