Job summary

Adara Home Health is seeking an Intake Coordinator to serve as a liaison to referral sources and facilitate provision of home care services. You will gather client information, coordinate referrals, and work with clinicians to start care, while maintaining professional relationships with payers, clients and staff.

The in-office role is Mon–Fri, 8 am–5 pm. Ideal candidates have healthcare intake experience, excellent communication, strong organizational skills, and the ability to navigate

Qualifications

• Minimum of three years health care experience; intake and home health experience.
• Knowledge of Medicare, private insurance and Minnesota Medical Assistance regulations.
• Ability to promote and maintain a positive attitude and encourage others.
• Excellent written and oral communication skills.
• Strong organizational skills and ability to work independently with minimal supervision.

Responsibilities

• Receives and coordinates all referrals, including notification of insurance providers for prior authorization and services covered.
• Assists prospective clients and referral sources in identifying appropriate services.
• Provides referral source with Company scope of service and coverage area.
• Maintains professional, positive relationships with payers, clients and employees.
