Wellbound Home Care is seeking an Intake Coordinator to be the primary contact for patients, families, referral sources, hospitals, and physicians. You will manage the intake and onboarding from referral to start of care, ensuring timely communication, accurate documentation, insurance verification, and coordination with clinical teams to admit patients smoothly.

• Minimum 1 year of experience in home care intake or healthcare coordination.
• Prior CHHA or home health experience preferred.
• Understanding of CHHA regulations and payer requirements.
• Familiarity with intake workflows and referral management.
• Excellent verbal and written communication; strong interpersonal skills.

Responsabilidades

• Receive, review, and process referrals from hospitals, physicians, and insurance providers.
• Verify required documentation and enter data into agency systems.
• Coordinate and schedule initial nursing and therapy assessments.
• Communicate with clinicians on patient needs and scheduling availability.
• Maintain accurate patient records in EHR systems and ensure regulatory compliance.
• Support workflow by daily intake meetings and team collaboration.
