Qualifications

• Proficiency with EMR/EHR systems and Microsoft Office Suite.
• Excellent communication, organizational, and multitasking skills.
• Compassionate, patient-focused demeanor with strong attention to detail.
• Bilingual English and Armenian.

Responsibilities

• Manage the end-to-end patient intake process, including referral intake, verification, and coordination of care.
• Communicate with physicians, hospitals, and referral sources to obtain necessary documentation and authorizations.
• Verify patient insurance eligibility and obtain prior authorizations for home health services.
• Coordinate with clinical staff to ensure timely and appropriate patient admissions.
• Maintain accurate and up-to-date patient records in the EMR/EHR system.
• Serve as a liaison between patients, families, referral sources, and internal care teams.
• Ensure compliance with Medicare, Medi-Cal, and private insurance guidelines during the intake process.
