Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Care Transition Coordinator to coordinate patient transitions from facilities to home health or hospice care. You will assess eligibility, plan care, and arrange services including DME and infusion, acting as liaison among the agency, referral sources, and healthcare providers.

The role includes executing outreach plans, managing sales-related admin tasks, educating patients, and ensuring timely documentation.

Qualifications

• Minimum of one year of experience in home health or hospital-based case management.
• Active RN/LPN/SW/PT licensure in the state of practice.
• Valid driver’s license and reliable transportation required.
• Experience with Medicare guidelines is preferred.

Responsibilities

• Achieve monthly production goals and MC admission targets for assigned locations.
• Implement weekly, monthly, and quarterly strategies to increase market share within facilities.
• Evaluate patients and physician orders for home care eligibility.
• Conduct face-to-face patient transitions and educate on plans of care.
• Complete encounter documentation in Home Care Home Base and coordinate transfers.
• Educate patients on post-discharge orders and services received.
• Collaborate with Executive Director and Clinical Director to drive growth.
