Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Care Transition Coordinator to streamline patient transitions from facilities to home health or hospice. You will assess eligibility, coordinate care plans, and ensure DME and infusion services align with patient needs.

You will liaise with referral sources and providers, drive market growth through outreach, and ensure compliance and patient education throughout the transition process.

Qualifications

• Minimum of 1 year of home health or hospital-based case management experience.
• Active RN/LPN/SW/PT licensure required in state of practice.
• Valid driver's license and reliable transportation.

Responsibilities

• Achieve monthly production goals and admission targets for assigned locations.
• Coordinate care plans and arrange DME and infusion services as needed.
• Act as liaison between agency, referral sources, and healthcare providers.
• Educate patients on post-discharge care and medical regimens.
• Document encounters and ensure compliance with agency policies.
