U.S. Renal Care, Inc. in Los Angeles, CA seeks a Home Therapy Registered Nurse to train patients and provide ongoing support for home dialysis modalities. You will conduct home visits, document care in EMR, and coordinate with physicians to optimize patient outcomes.

The role requires an active RN license, 12 months nursing experience with 3 months in the specific home modality, CPR certification, and strong communication and computer skills. On-call rotation and flexible locations are expected.

Qualifications

• Current RN license in applicable state.
• 12 months of clinical nursing experience in providing nursing care.
• 3 months of experience in the specific home dialysis modality.
• CPR certification required prior to patient care assignment.
• Ability to distinguish all primary colors.
• Must complete Home Therapy Nurse Orientation program and maintain competency.
• Proficient in English communication, both written and spoken.
• Basic computer skills; proficiency in USRC clinical applications within 90 days of hire.

Responsabilités

• Provide patient training and ongoing support for home dialysis patients.
• Perform home visits to assess patient home environment and care needs.
• Communicate assessment results to physicians and care team.
• Document nursing services in EMR with accurate reflection of patient status.
• Participate in infection control monitoring and reporting.
• Manage emergency preparedness and equipment procedures for home patients.
• Ensure medication management and ordering processes are accurate and timely.
• Coordinate home evaluations and ensure housing requirements for equipment operation.
• Assist in data collection for continuous quality improvement activities.
• Provide on-call nursing coverage nights and weekends as needed.
