Responsibilities include coordinating patient education, monitoring home environments, ensuring regulatory compliance, and participating in infection control and emergency procedures as needed.

Qualifications

• Current RN license in applicable state.
• CPR certification required prior to patient care assignment.
• Demonstrated ability to communicate in English, both written and verbal.
• Basic computer skills; proficiency in USRC clinical applications within 90 days of hire.

Responsibilities

• Provide patient training and ongoing support for home dialysis modalities.
• Perform home visits to assess patient environment and care needs.
• Document nursing services in the EMR with accurate, timely records.
• Coordinate with physicians and care team; communicate test results and patient status.
