Job summary

U.S. Renal Care, Inc. is seeking a Home Therapy Registered Nurse to train patients and support home dialysis care. You will perform home visits, assess environments, document nursing interventions, and coordinate with physicians to ensure high-quality, patient-centered care.

The role requires an active RN license, CPR certification, and strong English communication; computer literacy is expected. On-call shifts, weekend rotation, and travel to patient homes are part of the schedule.

Qualifications

• Current RN license in applicable state.
• CPR certification required prior to patient care assignment.
• Ability to communicate effectively in English, verbal and written.

Responsibilities

• Train patient (and/or care partner) in self-care for dialysis modalities.
• Conduct home visits to assess and improve the home environment.
• Document nursing services in the EMR with accurate status and interventions.
• Coordinate with physician and care team for ongoing patient monitoring.
• Participate in infection control monitoring and reporting.
• Provide on-call nursing services, including nights and weekends on a rotation.
• Arrange supplies and manage inventory to support patient prescriptions.
