U.S. Renal Care, Inc. is seeking a Home Therapy Registered Nurse to train patients for home dialysis and provide ongoing support. You will conduct home visits, document in the EMR, and coordinate with physicians to ensure high-quality patient care.

Responsibilities include infection control, emergency preparedness, and inventory management, with on-call duties as needed. A valid RN license, CPR, and strong communication skills are required to support safe patient outcomes.

Qualifications

• Active RN license in applicable state.
• 12 months of clinical nursing experience with 3 months in the specific home dialysis modality.
• CPR certification required prior to patient care assignment.
• Ability to communicate verbally and in writing in English.
• Basic computer skills; proficiency in clinical applications within 90 days of hire.

Responsabilités

• Train patient (and/or care partner) in home dialysis self-care practices.
• Conduct home visits to assess the patient’s home environment and care needs.
• Document nursing services in the EMR with clear and complete notes.
• Monitor infection control and emergency preparedness procedures.
• Coordinate supply inventory and timely orders to support patient prescriptions.
• Obtain routine and non-routine lab tests and communicate results promptly.
• Perform on-call nursing services, nights and weekends on a rotation.
• Maintain attendance and flexibility to accommodate program needs.
