Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to provide patient training and ongoing support for home dialysis modalities. You will perform home visits, document nursing services in the EMR, and communicate results to the physician and care team.

The role requires an active RN license, CPR, and 12 months of nursing experience with at least 3 months in home dialysis. Expect on-call rotation and travel to patient homes as needed.

Qualifications

• Current RN license in applicable state.
• 12 months of clinical nursing experience plus 3 months in the modality of home dialysis.
• CPR certification required before patient care assignment.
• Ability to distinguish all primary colors.
• Complete Home Therapy Nurse Orientation and maintain annual competency.
• Basic computer skills; proficiency in USRC clinical applications within 90 days.

Responsibilities

• Train patient and/or care partner in home dialysis self-care.
• Conduct home visits to assess home environment and patient performance.
• Document nursing services in the EMR and communicate results to physician and team.
• Participate in infection control monitoring and emergency preparedness procedures.
• Coordinate water sampling and home evaluation with technical services for equipment operation.
• Perform on-call nursing duties nights and weekends as needed.
