Job summary

Fresenius Medical Care seeks an entry-level Home Therapies RN CAP 1 to join the End Stage Kidney Disease care team. You will assess, plan, and coordinate home dialysis training and treatment for an assigned patient group, following established protocols and communicating needs to the physician.

As part of the CAP, you will educate patients and families, support home dialysis candidates, and participate in quality improvement activities while developing into a CAP-qualified professional.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state RN licensure.
• CPR/BLS certification.
• Minimum 12 months RN experience.

Responsibilities

• Provide and coordinate clinically competent care for assigned patients.
• Assess and educate patients and families about home dialysis training and treatment.
• Administer medications per prescribed protocols and document effectiveness.
• Train Home Dialysis patients and Home Partners on equipment operation and supplies.
• Collaborate with interdisciplinary team to ensure quality of care and timely lab processing.
