Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP positioned to coordinate and deliver comprehensive home dialysis care. You will assess, plan, implement, and evaluate patient care under supervision, and work closely with the physician and interdisciplinary team.

The role includes training patients and families for home dialysis, documenting care plans, and supporting quality improvement. RN licensure and CPR/BLS certification are required.

Qualifications

• RN with current state licensure.
• CPR/BLS certification.
• 1+ year RN experience preferred; entry level CAP pathway.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation.
• Educate patients and families on home dialysis training and treatment.
• Collaborate with interdisciplinary team to ensure safe delivery of dialysis therapy.
• Document nursing assessments and plan of care; adjust as needed.
• Participate in on-call rotation and quality improvement activities.
