Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP 1 to coordinate and deliver home dialysis care. You will assess patients, plan and evaluate care, and communicate with the physician as needed.

As part of the End Stage Kidney Disease team, you will participate in teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations. Cap requirements to advance will be pursued as part of the program.

Qualifications

• Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse.
• Minimum of 1-year experience as a Registered Nurse (preferred)
• Home dialysis therapy experience (preferred)

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation.
• Document data and adjust treatment plans per protocol.
• Educate patients and families about home dialysis training and modalities.
• Deliver safe and effective dialysis therapy in collaboration with the care team.
• Participate in on-call rotation and quality improvement activities.
