Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP 1 to join our team. This role introduces you into the Clinical Advancement Program with accountability for coordinating and delivering clinically competent home dialysis care under established protocols.

You will assess patient responses, educate families, and collaborate with the interdisciplinary team to ensure safe dialysis delivery and positive patient outcomes in a home setting.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR/BLS Certification.
• Must meet the practice requirements in the state in which employed.

Responsibilities

• Provides and coordinates delivery of home dialysis care following prescribed protocols.
• Assesses and documents patient response to home dialysis training and treatment; communicates with physician as needed.
• Educates patient and family on home dialysis training and treatment.
• Directs home care staff to ensure safe and effective dialysis delivery.
• Administers medications as prescribed and documents effectiveness.
• Participates in on-call rotation and may travel to training sites or patient homes.
• Contributes to quality improvement activities as part of the care team.

Education

Graduate of an accredited School of Nursing

Current appropriate state licensure

CPR/BLS Certification

Must meet state practice requirements
