Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP to coordinate and deliver comprehensive home dialysis care. You will assess patient education needs, document nursing plans, and support patients and families in learning the therapy and equipment operation.

The role involves collaboration with the interdisciplinary team, manual and electronic charting, and adherence to CAP requirements to advance. RN licensure and BLS CPR are required.

Qualifications

• Entry level RN with less than 2 years of nephrology nursing experience.
• Minimum of 1-year RN experience (preferred).
• Home dialysis therapy experience (preferred).
• Graduate of accredited School of Nursing.
• Current state licensure.
• CPR BLS Certification.

Responsibilities

• Coordinate and provide clinically competent care for home dialysis patients.
• Assess and document patient and family education needs.
• Deliver and monitor safe delivery of dialysis therapy.
• Train Home Dialysis patients and Home Partners on equipment use.
• Ensure correct laboratory collection, processing and shipping procedures.
