Job summary

Fresenius Medical Care is seeking an entry level Registered Nurse for Home Therapies under the CAP program. This role provides and coordinates clinically competent care, including assessment, planning, intervention and evaluation for an assigned group of patients, focusing on home dialysis training and treatment and communicating patient issues to the physician.

Qualifications

• Entry level RN with less than 2 years of Nephrology Nursing experience.
• Minimum of 1-year experience as a Registered Nurse (preferred).
• Home dialysis therapy experience (preferred).

Responsibilities

• Performs essential nursing duties under supervision and guidance.
• Documents patient data and adjusts treatment plans as needed.
• Educates patients and families on home dialysis and care at home.
• Ensures safe and effective delivery of dialysis care within home setting.
• Maintains awareness of transplant and treatment modality options.
• Participates in staff training and quality improvement activities.
