Job summary

Fresenius Medical Care is seeking an Home Therapies Registered Nurse to educate and empower dialysis patients in both clinic and home settings. You will coordinate care, assess learning needs, and support CAP advancement while ensuring safe dialysis delivery and adherence to protocols.

The role focuses on training patients and partners, performing home dialysis training, and collaborating with the care team to optimize outcomes across shifts and locations.

Qualifications

• RN with nephrology background and entry-level CAP pathway experience.
• Home dialysis experience is preferred.
• CPR/BLS certification and active nursing license required.

Responsibilities

• Provide clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Assess and document learning needs; educate patients and families.
• Deliver safe dialysis therapy in collaboration with Home Therapy team and caregivers.
• Administer medications per protocol and document effectiveness.
• Participate in on-call rotation and respond to emergencies as needed.
• Train patients/partners on Home Dialysis equipment and processes.
