Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR/BLS Certification.

Responsibilities

• Provide dialysis care and education for home patients.
• Train Home Partners on equipment operation.
• Assess and document patient progress and labs.
• Coordinate with interdisciplinary team.
• Participate in on-call rotation and leadership tasks.
