Job summary

Fresenius Medical Care in Texas is seeking an entry-level Home Therapies RN CAP to educate and coordinate home dialysis training for patients and their families. You will work with a multidisciplinary team to assess patient needs, plan care, and ensure high-quality dialysis treatment in clinic and home settings.

You will train Home Dialysis patients and partners, document plans of care, and support CAP advancement. Occasional travel to training sites or patient homes may be required.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR/BLS Certification.
• Home dialysis experience preferred.

Responsibilities

• Perform essential functions under supervision and collaborate with Educator and RN staff.
• Collect and analyze dialysis data, adjust treatment plans, document in medical record.
• Provide patient and family education on home dialysis and treatment modalities.
• Ensure safe and effective delivery of dialysis therapy and monitor patient response.
• Train patients and home partners on home dialysis equipment and procedures.
• Participate in CAP requirements to advance professionally.
