Job summary

Fresenius Medical Care is hiring a Home Therapies RN CAP 2 who will educate and train dialysis patients for home peritoneal dialysis and home hemodialysis. The role includes coordination of care, teaching, and leading Home Therapies staff within the End Stage Kidney Disease health care team.

The position involves assessing patients, planning interventions, and ensuring quality outcomes in clinic and home settings, with participation in CAP requirements and on-site education.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR/ BLS certification required.

Responsibilities

• Provide and coordinate clinically competent care for an assigned group of patients.
• Educate patients and families on home dialysis therapies and treatment plans.
• Lead and mentor home therapy staff and participate in quality improvement activities.
• Administer medications as prescribed and document effectiveness.
• Collaborate with the interdisciplinary team to ensure safe delivery of dialysis in home or clinic settings.
