Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP to educate and train dialysis patients on peritoneal and home hemodialysis. The role primarily operates in the clinic setting with initial home visits alongside a staff member.

The RN is accountable for assessment, planning, intervention, and evaluation of care for an assigned patient group, collaborating with the physician as needed and contributing to quality improvement efforts.

Qualifications

• Graduate of an accredited nursing program and RN licensure.
• Current CPR/BLS certification and ability to meet state practice requirements.
• Entry level to 2 years nephrology nursing experience preferred.

Responsibilities

• Provide education to patients and families on home dialysis.
• Coordinate and deliver safe dialysis therapy with the care team.
• Administer prescribed medications and document outcomes.
• Collect and review dialysis data and adjust treatment plans.
• Participate in ongoing staff training and quality improvement.
