Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN to educate and support dialysis patients in both clinic and home settings. You will participate in the Clinical Advancement Program (CAP), coordinate care with the care team, and train patients in home dialysis procedures.

The role emphasizes patient education, safety, teamwork, and adherence to regulatory standards. Travel to patient homes may be required.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet state practice requirements.
• Entry level RN with less than 2 years nephrology nursing experience.
• Minimum 1 year RN experience (preferred).
• Home dialysis therapy experience (preferred).

Responsibilities

• Provide clinically competent care including assessment, planning, intervention and evaluation.
• Educate patients and families on home dialysis treatment modalities.
• Coordinate care with physicians, educators, and home care partners.
• Administer medications as prescribed or per algorithm and document effectiveness.
• Train patients and home partners in safe operation of home dialysis equipment.
• Participate in on-call rotation and travel to training sites or patient homes as needed.
