Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP to empower and educate dialysis patients in peritoneal and home hemodialysis. This role emphasizes teaching, care coordination, and hands-on training in clinic and home settings, with occasional travel and on-call duties.

The position focuses on evaluating patient responses, collaborating with physicians, and leading education efforts for patients and families while supporting continuous quality improvement in a healthcare

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state RN licensure.
• Current or successful CPR/BLS certification.
• Home dialysis experience preferred.

Responsibilities

• Assist with and coordinate clinically competent care per patient plan.
• Collect and analyze dialysis data and document in records.
• Provide initial and ongoing learning for patients and families.
• Deliver safe, effective home dialysis therapy with partners and staff.
• Administer medications per protocol and document effectiveness.
• Participate in emergency response and staff training.
• Ensure proper lab collection, processing, and timely rescheduling.
• Update nursing assessment and care plan with the care team.
• Educate patients on transplant and treatment options.
• Identify home environment barriers and home dialysis readiness.
• Train patients/partners on home dialysis equipment and supplies.
• Complete CAP requirements to advance and support coverage needs.
