Job summary

Fresenius Medical Care in Independence, MO, is seeking an entry-level Home Therapies RN CAP to train and educate dialysis patients on peritoneal and home hemodialysis, and to coordinate care with the clinical team.

The role includes hands-on home visits, patient assessment, education delivery, and supervising other staff as assigned; must have nursing degree, state licensure, CPR/BLS, and nephrology experience is preferred but not required. Training provided.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state licensure.
• Current or successful CPR/BLS certification.
• Must meet the practice requirements in the state of employment.
• Entry level RN with <2 years of nephrology nursing experience.
• Minimum 1 year RN experience (preferred).
• Home dialysis therapy experience (preferred).

Responsibilities

• Performs essential functions under supervision and guidance of Educator or RN supervisor.
• Collects and analyzes dialysis data and documents in patient records, adjusting plans as needed.
• Assesses and documents learning needs to provide initial and ongoing education for patients and families.
• Delivers safe and effective home dialysis treatment in collaboration with patients and care partners.
• Administers medications per prescribed protocols and documents justification and effectiveness.
• Initiates emergency response measures when required and participates in staff training.
• Provides leadership and training to other staff and participates in onboarding.
• Ensures proper lab collection, processing, and shipping; reschedules missed collections.
• Identifies outcomes and updates nursing assessments and care plans with the team.
• Educates patients about transplant options and home modality choices.
• Trains patients and partners on home dialysis equipment and supplies.
