Job summary

Fresenius Medical Care in the United States seeks a Home Therapy Registered Nurse to support dialysis patients with home peritoneal and home hemodialysis training. The role involves one-on-one education, patient assessment, and coordination within the ESKD care team.

The RN will conduct initial home visits, guide care partners, and collaborate with physicians to optimize treatment outcomes while adhering to CAP program requirements and safety protocols.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Performs nursing care under supervision with Educator, Preceptor or in collaboration with another Registered Nurse.
• Collects and analyzes dialysis data; documents in medical record; adjusts treatment plan as indicated.
• Educates patients and families on home dialysis training.
• Delivers safe and effective home dialysis therapy with care staff collaboration.
• Administers medications as prescribed or per approved algorithms; documents effectiveness.
• Participates in emergency response measures.
• Provides staff training and orientation as assigned.
• Ensures proper lab collection, processing and shipping; reschedules labs.
• Updates nursing assessment and care plan with Interdisciplinary Team.
• Informs patients about transplant and treatment options.
• Assists in evaluating Home Dialysis candidates and home readiness.
• Trains Home Dialysis patients and/or Home Partners on equipment and supplies.

Education

Graduate of an accredited School of Nursing
