Qualifications

• The role is an entry-level designation as Home Therapies RN CAP 1 within the clinical advancement program.
• Provides clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.

Responsibilities

• Performs duties under supervision with Educator, Preceptor or another RN.
• Collects and analyzes dialysis data for assigned patients and documents in the medical record.
• Assesses and documents patients' learning needs to provide initial and ongoing education for patients and families.
• Directs home dialysis care with the patient, home partner, and care staff to deliver safe dialysis therapy.
• Administers medications per prescribed protocol and documents justification and effectiveness.
• Participates in emergency response measures when needed.
• Serves as a resource for the care team and assists in staff training and orientation.
• Ensures proper lab collection, processing, and shipping and reschedules missed labs.
• Updates nursing assessment and care plan in collaboration with the Interdisciplinary Team.
• Educates patients about transplant and treatment modality options.
• Identifies barriers in home environment and readiness for home dialysis.
