Job summary

Fresenius Medical Care is seeking an Home Therapy Registered Nurse to train and support dialysis patients in home therapies. The role involves one-on-one patient education, clinical assessment, and coordinating care within the End Stage Kidney Disease team.

You will work with a team to prepare patients for home dialysis, perform initial home visits, and ensure safe, effective treatment in the home setting. Entry-level RN with valid state license and CPR/BLS is required, with preference for home

Qualifications

Graduate of an accredited School of Nursing.
Current appropriate state licensure.
Current or successful CPR BLS Certification.
Must meet state practice requirements.
Entry level for RNs with less than 2 years of nephrology experience.
Minimum 1 year RN experience (preferred).
Home dialysis experience (preferred).

Responsibilities

Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
Educate patients and families about home dialysis options and care plans.
Train Home Dialysis patients and partners on equipment operation and safety.
Perform essential nursing functions under supervision with guidance from Educator or RN.
Document dialysis data and update care plans in the medical record.
