Job summary

Fresenius Medical Care is seeking an entry level Home Therapies RN CAP 1 to coordinate and deliver dialysis-related care and education to patients. You will assess responses to home dialysis training and treatment, document plans, and collaborate with a multidisciplinary team to improve patient outcomes.

The role requires an active RN license, CPR/BLS certification, and the ability to work with home dialysis partners.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state RN licensure.
• Current or successful CPR BLS certification.
• Must meet the practice requirements in the state of employment.
• Entry level RN with less than 2 years nephrology nursing experience.
• Preferred: 1 year RN experience; home dialysis experience preferred.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation.
• Assess patient response to home dialysis training and treatment per protocols.
• Educate patients and families on learning needs and care plans.
• Administer medications per protocol and document effectiveness.
• Participate in on-call rotation and quality improvement activities.
