Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a Physical Therapist to provide functional rehabilitation in home and community environments. You will evaluate patients, develop and adjust treatment plans, and collaborate with a multidisciplinary team to support real-life activities like cooking, shopping, and workplace tasks.

The role emphasizes autonomous work within a collaborative team, ongoing assessment, and education for patients and families.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted PT license in the state of practice.
• Current BLS/CPR certification as required by state regulations.
• At least 1 year of PT experience preferred.
• Knowledge of rehabilitation techniques for neurological injury.
• Strong communication skills, professional conduct.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and participate in staff/family meetings.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team and family.
• Collaborate with other disciplines to coordinate care and improve outcomes.
