Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced Physical Therapist to deliver functional, real-world rehabilitation in home and community settings. You will design and implement therapy plans tailored to individuals’ daily life activities, from kitchen tasks to community mobility.

Ideal candidates have a bachelor’s in PT, active state licensure, and at least one year of PT experience, with strong communication and a commitment to patient-centered outcomes.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current BLS/CPR certification as per state regulations.

Responsibilities

• Perform evaluations and develop treatment plans for patients.
• Regularly reassess treatment effectiveness and hold team/family conferences.
• Perform discharge evaluations and provide home safety recommendations.
• Communicate patient needs and progress to the care team and family.
• Collaborate with other disciplines to coordinate care and improve outcomes.
