Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a Physical Therapist to deliver evidence-based care in patients’ homes and communities. You’ll work with an interdisciplinary team to develop innovative treatment plans that promote independence and improved mobility.

The role offers clinical autonomy, flexible scheduling, and the opportunity to impact patients in real-world environments, from daily activities at home to community participation and work readiness.

Qualifications

• Minimum bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support (BLS) or CPR certification per state regulations.
• Minimum of one year’s PT experience preferred.
• Knowledge of rehabilitation techniques for complex neurological injury.
• Excellent verbal and written communication skills.
• Ability to lift 50 pounds; more with safety procedures.
• Physical requirements include standing/walking for extended periods.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and attend staffing meetings.
• Perform discharge evaluations and provide home and community safety recommendations.
• Communicate patient needs and progress to the treatment team and families.
• Coordinate with other disciplines to enhance patient outcomes.
