Job summary

Rehab Without Walls Neuro Rehabilitation, affiliated with BrightSpring Health Services, seeks a Physical Therapist to provide functional rehabilitation in home and community settings across life activities. You will assess, plan, and re-evaluate treatments, supporting patients in real-life environments such as kitchens, stores, and workplaces.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapy license in the practicing state.
• BLS or CPR certification in accordance with state regulations.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and participate in meetings.
• Conduct discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team and family.
• Collaborate with other disciplines to improve patient outcomes.
