Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a licensed Physical Therapist to deliver home and community-based rehabilitation services. You will evaluate, treat, and create plans of care for clients in their homes, schools, workplaces, and community settings.

You will work autonomously within a collaborative therapy team, conducting re-assessments, discharge planning, and coordinating with families and physicians to improve functional outcomes.

Qualifications

• Minimum bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current BLS/CPR certification in accordance with state regulations.
• At least one year of PT experience preferred.
• Knowledge of neurorehabilitation techniques for complex injuries.
• Strong verbal and written communication; teamwork skills.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and attend meetings.
• Conduct discharge evaluations and provide home safety referrals.
• Communicate patient needs and progress to treatment team and family.
• Collaborate with other disciplines to improve outcomes.
