Job summary

Rehab Without Walls Neuro Rehabilitation, part of BrightSpring Health Services, is seeking a Physical Therapist to deliver in-home and community-based rehabilitation. You will evaluate, treat, and re-evaluate clients in real-life settings such as kitchens, stores, and workplaces, ensuring logical progress toward functional goals.

Role emphasizes autonomous work within a collaborative therapy team, with opportunities for growth and advancement, and a flexible schedule tailored by you.

Qualifications

• Minimum of bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist by state of practice.
• Current Basic Life Support or CPR certification as required by state regulations.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make recommendations for home and community safety, and provide resources.
• Communicate patient needs and progress to the treatment team, physician, and family members.
• Collaborate with other disciplines to enhance patient outcomes.
