Job summary

Ambercare/Addus HomeCare in Roswell, NM is seeking a Service Coordinator to lead caregiver scheduling, ensure high-quality service, and conduct in-home visits.

You will be the go-to problem solver, juggling client updates, supporting field staff, and keeping care plans on track in a fast-paced environment where making a real difference matters.

Schedule: Monday through Friday, 8am–5pm, with a 1-hour lunch and on-call rotation on weekends. A comprehensive benefits package is offered.

Qualifications

• Must have high school diploma or equivalent.
• 4 years of office experience.
• Interpersonal, organizational and communication skills.
• Computer skills including Microsoft Word, Microsoft Excel and Scheduling program.
• Must have reliable transportation and a valid DL for travel to home visits monthly and some marketing.

Responsibilities

• Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence.
• Provides alternate coverage to ensure the client’s care plan is followed and client services are not interrupted.
• Contacts care providers and clients to provide service updates.
• Conducts monthly client wellness calls and conducts home visits as required.
• Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company’s policies and guidelines.
• Maintains a high degree of confidentiality at all times due to access to sensitive information.
• Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements.
