Ambercare – an Addus family company is seeking a Full Time Service Coordinator in Albuquerque, NM. The role focuses on scheduling and supervising in-home care workers and clients within a geographic area, including conducting in-home visits to verify service quality and adequacy of care plans.

The position requires coordinating schedules, ensuring coverage, and maintaining compliance with Medicare, Medicaid, and HIPAA regulations while coordinating with care teams and clients.

Qualifications

• 2–5 years caseload or management experience.
• Experience with scheduling and/or case management preferred.
• Proficient in Microsoft Word, Excel, and Outlook.
• Strong communication and interpersonal skills.

Responsibilities

• Creates work schedules by entering schedules into the system; manages changes due to requests, illness, vacation or leaves of absence.
• Provides alternate coverage to ensure care plan is followed and services are uninterrupted.
• Contacts care providers and clients to provide service updates.
• Monitors client condition and performance of Home Care Aides.
• Maintains compliance with regulations and standards (Medicare, Medicaid, HIPAA).
• Conducts home visits to welcome new clients and review Welcome Packets.
• Assesses Home Care Aides in clients’ homes to ensure Care Plans are followed.
• Prepares accurate evaluation reports and case notes as needed.
• Maintains schedules to ensure timely completion of all assigned visits.
• Attends weekly branch meetings and maintains confidentiality.
