Job summary

Community Health Network, Inc. is seeking an experienced Home Health RN Case Manager to support patients in the Cambridge/Watertown, MA area. The role focuses on coordinating home health services, completing OASIS visits, and ensuring optimum patient outcomes within a Medicare-certified agency.

Coverage needs primarily in and around the Cambridge/Watertown, MA area and surrounding towns.
