Job summary

Halcyon Home LLC is seeking a charismatic Home Health Physical Therapist Assistant (PTA) to join our fast-growing geriatric home health team in Northeast Austin and nearby communities. This PRN position supports flexible scheduling with a strong home health background preferred.

Current PTA license required; Homecare Homebase EMR knowledge is a plus. You will assess patients, develop care plans, educate families, and coordinate with physicians while delivering compassionate patient care in

Qualifications

• Current PTA license.
• Knowledge or experience using Homecare Homebase EMR is a plus.
• Experience in home health or related healthcare setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Performs ongoing clinical assessment including OASIS at appropriate time points.
• Develops and evaluates the plan of care with patient and caregiver(s).
• Documents notes, orders, and records in a timely manner; revises POC based on data.
• Accepts assignments within education and competence.
• Provides supervision to PTAs and home health aides as appropriate.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
• Communicates with physicians and other health care practitioners about the plan of care.
• Meets continuing education requirements.
• Instructs patient and family in transfer techniques and use of equipment.
• Provides patient and caregiver education and counseling.
• Oversees team members where applicable and follows infection control protocols.
