Job summary

Optimal Care is seeking a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on high-quality, patient-centered care.

You will conduct medical evaluations, develop treatment plans, prescribe/monitor medications, and collaborate with physicians and the broader care team to prevent hospitalizations and improve quality of life.

Qualifications

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant is required.
• Current State License as NP or PA is required.
• Reliable transportation, valid driver's license, and valid automobile insurance coverage.

Responsibilities

• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications and coordinate with physicians and care team.
• Serve as a key clinical decision-maker and help patients understand diagnoses.
• Manage chronic illness and avoid unnecessary hospitalizations.
• Coordinate with caregivers and interdisciplinary team to support patients at home.
