Job summary

Ennoble Care is seeking an Advanced Provider (NP/PA) to deliver primary and palliative care to homebound patients in the Howard County, MD region on a full-time basis. You will apply medical principles under physician supervision and care for a diverse patient population with chronic conditions.

You will assess health via exams, histories, and care planning, collaborating with a multidisciplinary team. A Maryland license and 1–2 years of geriatric primary care experience are required; hospice

Qualifications

• Board Certified NP or PA licensed in Maryland.
• 1–2 years of primary care experience with geriatric patients.
• Hospice experience preferred but not required.

Responsibilities

• Provide high quality primary and palliative care to homebound patients in Howard County.
• Perform physical examinations, review histories, and create care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Deliver patient-centered care to a panel of patients.
• Develop impressions, diagnoses, and treatment plans.
• Build trust with patients, families, and caregivers during visits.
• Order diagnostic testing as needed.
• Prescribe medications and medically necessary interventions.
