Job summary

Ennoble Care in Baltimore, MD is seeking an experienced Advanced Practice Provider to join our full-time team serving primary and palliative patients at home. You will apply medical techniques under physician supervision and care for a diverse group with chronic diseases.

Requirements include board certification and Maryland NP/PA credentials, strong clinical skills, independence, and 1–2 years in primary care with geriatric patients (hospice experience preferred).

Qualifications

• Board Certified, with a valid NP or PA certification in Maryland.
• Strong clinical skills with the ability to treat, assess, and diagnose.
• Positive demeanor and strong people skills.
• Ability to problem solve and operate independently.
• 1-2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality care to primary and palliative patients in and around Baltimore.
• Assess health by performing physical exams, obtaining histories, and creating care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Provide high quality, patient-centered care to a panel of patients.
• Develop impressions, diagnosis, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary.
• Prescribe medications, supplies and appropriate interventions.
