Job summary

Ennoble Care is expanding in Baltimore County, MD, and seeks an experienced Advanced Provider to deliver primary and palliative care in patients' homes. You will work under physician supervision to manage a diverse chronic-disease panel and ensure high-quality, patient-centered care.

The role requires MD-certified NP or PA with 1–2 years of geriatric primary care; hospice experience desired. Full-time position with comprehensive benefits and growth opportunities within a growing team.

Qualifications

• Board Certified, with a valid NP or PA certification in Maryland.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality care to primary and palliative patients in and around Baltimore County.
• Assess patient's health by performing physical examinations; updating and studying histories and creating care plans.
• Communicate with office staff and clinical operations across internal platforms
• Provide patient-centered care to a panel of patients
• Develop impressions, diagnosis, and treatment plans
• Consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications and medically necessary interventions
