Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in patients' homes, including medical history review, diagnosis, and follow-up. The role involves collaboration with primary care physicians, case managers, and other healthcare professionals, with testing and documentation done per Matrix guidelines.

Home-based visits offer flexible scheduling and potential growth opportunities.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority or ability to obtain
• AANP/AACN/ANCC board certification in Adult/Adult Gerontology/Family/ Emergency/etc.
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred, new grads encouraged to apply
• Valid state driver’s license
• Automobile insurance coverage for state of residence

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family members and other providers
• Coordinate with PCP on patient education and follow-up
• Provide services at the member’s residence (home or Skilled Nursing Facility)
