Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments. You’ll review medical history, medications and social factors, then coordinate with PCPs and care teams to close care gaps. The role emphasizes patient education, documentation, and adherence to Matrix protocols.

Flexibility is offered with scheduling, including weekends. Prior NP licensure, board certification, and valid driver's license are required; EHR proficiency and ongoing education

Qualifications

Master’s degree in nursing or commensurate NP experience.
Current NP licensure in state of practice with prescriptive authority.
Board certification in Adult/Family/ gerontology specialties.
BLS/ACLS/CPR certification.
Valid state driver's license and auto insurance.
Strong computer skills and familiarity with health records software.

Responsibilities

Conduct adult/geriatric assessments, document findings per Matrix guidelines.
Administer order point-of-care and lab tests as needed.
Collaborate with physicians, case managers, social workers and caregivers.
Provide care at member homes or Skilled Nursing Facilities; follow up education.
