Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform in-home comprehensive health assessments. Each visit covers health history, risk assessment, education, and follow-up planning, with careful documentation per Matrix guidelines.

Providers administer tests as needed and work with a care team to address gaps in care, offering services at members’ homes or SNFs while maintaining strong patient communication.

Qualifications

• Master’s degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in Adult, Family, or gerontology specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged.
• Valid state driver’s license; auto insurance as required.
• Strong computer skills and familiarity with health records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments, medical history, diagnosis, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on education and follow-up.
• Provide services at members’ homes or in SNFs.
